Merchant’s Business and Trade Name: The official registered business and, if applicable, trade name of your establishment.
Contact Details: Primary phone number and active business email address for operational alerts and settlement notifications.
BIR Form 2303: Official Bureau of Internal Revenue Certificate of Registration confirming tax status and TIN verification.
B. Operational Permits & Menu Setup
Mayor’s / Business Permit: A valid, unexpired permit issued by the local government unit (LGU) where the store operates.
Sanitary Permit: Valid health clearance ensuring compliance with local food handling and hygiene standards.
Complete Menu / Product List: Detailed list of food items, prices, descriptions, and thumbnail photos for store configuration.
C. Financial & Legal Authorization
Bank Payout Details: Valid bank account information (account number, bank name, passbook/bank statement) for automated payouts.
Government-Issued Photo ID: Clear photo identification (e.g., UMID, Passport, Driver's License) of the primary owner or authorized representative with specimen signature.

3. Merchant Registration Form (MRF) & Store Activation
Once all primary and entity-specific documents have been uploaded and verified, the final administrative milestone is the Merchant Registration Form (MRF):
Mandatory Requirement: All business types (Sole Proprietorships, Partnerships, and Corporations) are required to sign and submit the our MRF.
Legal Binding & Terms: The MRF formalizes commission rates, operational guidelines, settlement terms, and service standards.
Final Activation Trigger: Submission of the signed MRF triggers final store system checks, enabling the establishment to start receiving orders on the ShopeeFood app platform.
